Participating Sites

The three group practice sites selected to participate as demonstration sites in the PIN Initiative were:

  • Agassiz Medical Centre in Morden, Manitoba
  • Dr. C. W. Wiebe Medical Centre In Winkler, Manitoba
  • Assiniboine Clinic In Winnipeg, Manitoba

Brandon Medical Clinic was a fourth practice selected but chose to withdraw from the demonstration phase.

Steinbach Family Medical Centre was selected as a Control Site for the PIN Initiative. They have now converted to a full PIN site as of January 2009 (PIN Phase 2).


PIN Phase 1 Demonstration Sites Activities

During Phase 1, the three demonstration sites implemented practice changes within their clinic.

Dr. C.W. Wiebe ClinicDr. C.W. Wiebe Clinic in Winkler concentrated its efforts on preventative practices and coronary artery disease (CAD). The clinic hired a nurse and a dietitian to assist them in meeting their PIN Objectives (e.g. health education, ensuring patients have tests and screenings done, creating a notification system to remind physicians and patients when certain tests are required).

Assiniboine Clinic in Winnipeg focused their efforts on hypertension and diabetes. The clinic hired a diabetes nurse educator and a foot care nurse to help provide care to patients with diabetes and hypertension.

Agassiz Clinic in Morden also focused their efforts on hypertension and diabetes. The clinic hired a dietitian to work with patients with diabetes and other chronic diseases. Agassiz also strived to ensure their nursing staff worked to their full scope of practice by hiring an administrative staff person to decrease the amount of administrative work nurses are responsible for.

Steinbach Family Medical Centre implemented information management changes and used their EMR to begin collecting information on clinical process indicators. In Phase 2, Steinbach Family Medical Centre converted to a demonstration site, like the initial three demonstration sites, and will begin implementing practice change. 


PIN Phase 2

PIN Phase 2 officially began September 1st, 2008 when the Phase 1 demonstration sites began their second year of Quality-Based Incentive Funding (QBIF) .
