Referrals:

Referrals to Community Rehab Services may be made by any of Selkirk Mental Health Centre’s treatment teams. Individual programs are designed with the patient to assist them in developing specific skills in order to meet their identified goals.  Regular reviews are conducted and information is shared with the treatment team, including community mental health personnel, as a means of facilitating the return to the community setting of the patient's choice.

Process:

Once accepted, patients participate in activities offered by Community Rehab Services which seeks to facilitate a seamless transition from inpatient treatment unit, to a Community based living situation. Individuals remain patients of Selkirk Mental Health Centre, and in effect are transferred to Community Rehabilitation Services in a manner similar to other transfers that occur between in-patient treatment areas.

Communication between Community Rehabilitation Services and the treatment team takes place at Treatment Team meetings and on an ongoing, as needed basis when pertinent issues arise.  The mutual support/cooperation between all members of the treatment team is seen as vital to the provision of an effective, patient-centered service.A Community Rehabilitation Services residence in Selkirk

Typically, a patient will begin by participating in a day program, which provides the opportunity for assessment and training according to their own goals and at a pace, which they feel comfortable.  The progression then is to overnight stays and further adjustments in time spent at Community Rehabilitation Services according to the patient's ability and personal growth.  Eventual progression then can take place from the supervised setting, to the more independent situation.

As the patient achieves a full time placement, there is a corresponding increase in the focus on connecting with community resources, such as Community Mental Health Services, Support Groups, Social Assistance Workers and others.  In so doing, the opportunity to realize one's full potential for successful transition to life away from Selkirk Mental Health Centre is maximized.
