Well Sealing Report - PDF version

A PDF version of the well sealinng report is now available for use.
This format is an alternative to completing the traditional paper version.
The PDF version allows you to complete and save the report on your computer.
You are also able to print a copy of the completed file which can then be
provided to the Groundwater Management Section, to your client, and be kept
for your own filing purposes.

Computer requirements
Users are encouraged to use Adobe Acrobat Reader which may be pre-installed
on your computer or can be downloaded from https://get.adobe.com/reader/
The suggested optional offers are not required (ex: Google Chrome and the
Google Toolbar). Other PDF readers can be downloaded for free on the internet,
but have not been tested to ensure they open the file correctly.

How to use the PDF file

Step 1  Open the file(attached):
Note the Clear, Save and Print buttons in the upper left hand corner of the file.
These buttons function as follows:

Clear clears (deletes) information from all the fields in the form.
Save allows the file to be saved. If an Error! message appears,
		click OK, and continue.
Print allows the form to be printed from your printer. If an Error!
message appears, click OK, and continue.

Toggle the Highlight Existing Fields button (in the top right hand corner)
to show or hide data fields as blue areas with red boundaries identifying key
mandatory fields.

Step 2 Enter the well sealing information.
Simply type information into the blue fields, or click on boxes where choices
are available. The well sealing report should be filled out using the
attached well sealing guide as a reference.

Step 3 Save the file.
Certain fields within the well sealing report have been earmarked as
mandatory, a reminder that the report must contain all the required information
specified in the Groundwater and Water Well Act. The file, however, can be
saved before it is completed despite an Error! message regarding missing mandatory content.
Choose a suitable filename when saving the file. Consider using the date
(year-month-day) the well was constructed, the well owners name and well ID tag number
(once the well ID tags are available), for example: 2017-01-29-John Smith-Tag 123456.

Step 4 Print & Mail.
At this time the PDF file cannot be submitted by e-mail. Rather, the file must be
printed, signed and mailed. The report must be printed on legal sized paper
(8.5 x 14 inches) and signed in ink. This size is required for review and filing
purposes. Reports submitted on letter sized paper (8.5 x 11 inches) will not be accepted.
