Reports are done annually on the previous year and are due on October 31, 2026. The guidelines for completing the report are included in the report template:

There are two ways the HSG Report can be completed and submitted:
- Email – complete the report in “word document” format OR complete and scan, then email it to healthyschools@gov.mb.ca
- Mail – complete the report and mail a hard copy to the Healthy Schools Initiative (4089-300 Carlton Street, Winnipeg MB R3B 3M9). Please keep a copy for your records before mailing.
